[phpesp-general] Unable to create survey

Hi,

I am having some problems with my phpesp. Everything well and fine on my 
development machine.
However, after migration to the test server, I can't seem to get past 
the create survey 'general' tab.
It simple re-route me by to the manage page and no records are created 
in the db.

somebody help please...
