[phpesp-general] Survey Design: can't use continue button

Dear members of this list

I tried phpESP the first time on this Demo:
http://phpesp.sourceforge.net/demo/admin/manage.php

However, when making a new survey, then hitting the "continue"
button (to go to the question forms) nothing happens.

I did also make a test-install on a server. Here the same issue.

I filled in all the "required" fields.

Do I miss something?
