Summary: scripts/phpespmod.pl has misleading comments

Initial Comment:
The comments describing how to compute the survey number and the question number are misleading, specifically referring to the wrong tables and forcing the user to do math.  These comments are also buried in the script, rather than being at the top.  Move the comments to the top and fix them.
