[phpesp-dev] patch: Purge access when purging survey

All,

Survey purge purports to remove "all traces of a survey from the  
database."  However, survey_purge() does not remove access control  
values defined in the access table.  These access control values  
should also be removed.

See attached patch.  This should be applied *after* the patches I sent  
previously for collecting survey statistics.
