[phpesp-general] handler-prefix.php error phpESP 2.0.2

We are upgrading to phpESP 2.0.2, and I have created a three page test
survey and a one page test survey. When testing, on the three page
survey when you complete the first page and go to the two page or on the
one page survey when you submit the survey, you get this error:
