#215 demo: cannot create new survey

Creating a new survey does not work in the online demo.

Replication:
- click on "New Survey"
- fill in all fields marked "Required"[red text]
- click "Continue"

Expected result:
- a new page

Actual result:
- same page, buttons on top to navigate to other sections do not work

Severity:
Highest severity: application is useless without ability to make new surveys
