What do I add to my web page?

I've downloaded and installed PHPCOUNTER on my PHP local server as instructed in the ReadMe.txt file but it makes no mention of what I have to put in my web pages in order to a) trigger an increment on the server database and b) display a counter for that page on the web page.

Have I missed something here or is there part of the ReadMe.txt that I am missing?
