#90 all newly created issues should be mailed to ADMIN_EMAIL

I do not fully understand when who gets a mail from phpbt.

I would find it useful, if newly created issues are
mailed to ADMIN_EMAIL and reporter.
All changes should be mailed to reporter and when a bug
is assigned it should be mailed to that user as well.

can there be a situation that the php based mail
sending does not work at all? I don't receive any mail
from phpbt at all. I verified that the ADMIN_EMAIL work
and my users mail do work as well. All users have
email-adresses in the database. I do use the latest
version 0.9.1 and are using it since 0.6.X or 0.7.X (do
not remeber exactly).
