At least on my Redhat 6.2 box with perl-5.00503,
perltidy 20010701 is broken.

The error is:

Can't define "_SPACES" as constant (name contains
invalid characters or is empty) at
/usr/local/bin/perltidy line 7332
BEGIN failed--compilation aborted at
/usr/local/bin/perltidy line 7332.

It looks like constants cannot begin with '_' in
5.00503 - I renamed all the constants (and their uses)
as x_SPACES etc. and perltidy seemed to work OK.

replacing them worked for me too.
For those who are looking for a quick fix:
open perltidy in vi and do:
    ESC :%s/\[_/[x_/g
    ESC :%s/\{_/{x_/g
    ESC :%s/\ _/ x_/g
    ESC :wq
