Pod::Simple::WIki, http://search.cpan.org/dist/Pod-Simple-Wiki/lib/Pod/Simple/Wiki.pm
and it's CLI tool pod2wiki: http://search.cpan.org/dist/Pod-Simple-Wiki/bin/pod2wiki
It is one of my favorite tools I use it with: Confluence, Twiki and madiawiki (SourceForge).
You have to be aware of how it handles links, but in general it is a recommendable tool.
