Branch to evaluate patche from Danny Sadinoff

Enclosed is a patch to add a little bit more control over the 
1) the name of the initial state in a workflow
2) the contents of the initial history record inserted at workflow-create time.  

I implemented #1 via config, and #2 via methods-to-override in Persister.pm.  I welcome any suggested resolution to this inconsistency.

Implemented a primitive test for #1, but not for #2.
