It sounds like a... dubious idea to me :)
But try this: in the main event loop in Tk (if there is one), call
Win32::GUI::DoEvents();
Or if there isn't one, try rolling your own main event loop in Win32::GUI
and call both Win32::GUI::DoEvents() and the Tk analogue. Or add a
Win32::GUI::Timer control to call the Tk::DoEvents() (or whatever it's called).

What do you mean by custom widget?
If you want to draw the contents of your own control, there is the
Win32::GUI::Graphic control which has a Paint event.
