#11 Enhancement of register handler

Attached, a patch that enhances the handling of registrations, including:
- introduction of "Expires" header (including default).
- addition of unregister all (wildcard)
- improved separation of functionality - RegisterHandler handles REGISTERs under control of InitialRequestManager
