#24 Implement destination lookup in action dictionary of outline
This patch adds to the outline generation capability the ability to examine the "Action" ("A") dictionary, then pull out the "D" (for DESTINATION) associated with that action. For some reason we have a whole heap of documents like that (that I have to pull images out of)
