#60 Split on multiple bookmark levels
Currently a single integer indicates the bookmark level for document splits.
I suggest a "default", integer "0" indicating that each bookmark creates a page.
Where multiple bookmarks exist on a page, multiple pages are generated.
To help with unique file naming, use [CURRENTPAGE]_[BOOKMARK NAME] as the naming method when integer "0" is used.
The most useful feature in my context is providing a file for each bookmark in the the original PDF.
