This is definitely a bug. The document you have provided (and I can
reproduce that with others too) has rotated pages - the first page is
rotated by 270 degrees - and the added text is added with in the
coordinating system without rotation (PDF calls it user space).

I have reported that as http://pdfedit.petricek.net/bt/view.php?id=369,
but unfortunatelly I have no idea how to fix this.

gs still makes something to the doment. I have tried:
1) add some text to the first page and save
2) gs -sDEVICE=pdfwrite -dNOPAUSE -dQUIET -dBATCH -dFirstPage=1 -dLastPage=7 -sOutputFile=/home/miso/tmp/test_out.pdf ~/tmp/test.pdf

The resulting document had a different orientation on the first page -
it looks like gs droped it so it looks like the value wasn't set before
and the added text has the same orientation wrt. the rest of the text,
just rotated.
