The X window is created at the size specified by -geometry, but the LINES and COLS macros still expand to 24x80, as do the getmaxx() and getmaxy() macros. Try ./testcurs -geometry 100x100.
How does one find the actual screen size?
I guess I can answer my own question: -lines and -cols are decoded and set the geometry of the X window.
Should -geometry not also be decoded to set the window size? After all you need -geometry to specify where the screen should be on the display; a discrepancy between -lines/-cols and -geometry would not be desirable?
