All, Well this morning I decided to build the new revision after Rony checked in my changes. Same result as last night, no toolbar buttons. But this time I decided to try the application resize feature (which is great!). Lo and behold, after I made the application slightly wider to show the password column the toolbar buttons appeared. Now I can't get the application to come up without showing those darn buttons no matter how small I make the window. I still don't know what is going on, but I'm a lot more confident that my changes did not break existing code. Maybe someone else has an idea of the root cause.
