Installed v3.41 and created new entries for my work accounts. If I'm already on the given URL autotype it works, but if I use GO+Autotype it does not. Far as I can tell nothing happens to send input. I'm using Firefox 47. One of the URLs was redirecting, if that matters, but the other one was not. My outlook web login didn't work with autotype either, perhaps because there is a radio input and a checkbox above the username/password, no amount of tabs or other options worked,

You have to design your autotype sequence to work for the website.
If "Browse to URL + Autotype" is what you are using, see a recent related discussion about it at https://sourceforge.net/p/passwordsafe/discussion/134800/thread/11f5109c/?limit=25#3fce. You may want to stick to first "Browse to URL" and then when the website is loaded and ready for input, send your Autotype or use the dragbar.
