#22 Add Document object to manage multiple images/image sets

We should add a high-level Document object to manage windows and resources specific to a given image; this will allow us to support multiple simultaneous images with far greater ease.

This is a fairly extensive change, as many (most?) classes will have to be changed to be document-aware.
