#264 mod_auth_pam does not set PAM_TTY

Hi,

apparently mod_auth_pam does not set PAM_TTY. As a
result pam_securetty.so cannot determine the tty and
the auth requests fail for "root".

The proposed patch just sets PAM_TTY to "hhtpd", which
then can be added to /etc/securetty (which of course
needs to be readable by httpd ...).

Error handling should be added, but I have no clue on
how to use the ap_error_logging stuff.
