#261 mod_auth_pam: pam authentication is used over AuthUserFile

The documentation of mod_auth_pam says that if
auth_pam_module is loaded after auth_module pam
authentication is tried after AuthUserFile authentication,
but that is not correct, pam authentication is used
before, and if it fails other authentication is not tried. All
the fallthrough settings don't change anything either.

(This is with the Apache 1 module.)
