This patch enables support for a custom prompt. This
is particularly useful if pam_ssh is being used as the
first item in the auth stack.

Gentoo Linux patches the sources to make the default
prompt "Password:", so ironically one of the uses of
this patch would be to reset the prompt to "SSH
passphrase:"!

This also fixes an apparent bug where the newly
introduced allow_blank_passphrase option nukes the list
of SSH key files on OpenPAM systems.

    It turns out this is of limited use since PAM doesn't allow
    quoting of whitespace, so a prompt can only be one word.

    So the prompt stuff doesn't work with PAM, correct?
    So I'm going to close that Rejected soon.
