#1003 Parse CML HIN (PID-2) properly

Parse international student HIN from CML properly.

CML lab for international students have HIN in PID-2 (instead of PID-4). Format for HIN is 'MUddddddddd' where d is a digit.

create sample patient that matches the patient in the provided sample CML HL7, and upload the lab. Make sure the HIN is parsed and matches the patient.
