#4055 get assign issues to work in new ui for notes/cpp

There are some issues observed related to Assign issues for notes/ CPP.

1. Removing CPP assign issue before saving a note keeps removed record.

Steps to replicate:

1. Add CPP Risk factors note and save it.
2. Re-open the note
3. Assign issue and select 'remove' to remove just assigned issue
4. Sign and save cpp note
   Expected: If the assign issue was removed before saving the note it shouldn't be indicated.
   Observed: Assigned issue was removed before the note was saved but there is an indication that the issue was removed.

2. Notes Assign issues field is dynamic

Steps to replicate:

1. Enter search keyword into assign issues notes.
2. Select search result and scroll up a vertical scroll bar.
   Expected: Notes Assign issues field should be static
   Observed: Search result field is dynamic, it's moving up/ down when scrolled.

3. Error when try to re-save cpp note with assigned issue

Steps to replicate:

1. Open new cpp note
2. Assign issue to cpp note
3. Sign and save note
4. Re-open the cpp note and select Sign and save button
   Observed: Error pops up "An error occured while trying to save Issuenote".
