#4051 EChart button in showdocument.jsp doesn't go directly to the EChart

In the showdocument.jsp, when clicking on the EChart button, it doesn't go directly to the Echart but the user has to click again on the patient to go to the EChart
Wanted result: Go directly to the Echart.

This works if a demographic is attached but breaks when viewing an unmatched document since the demographic no is -1.

Also notice that this is not unique and affected is the msg,tickler, echart, master and appt history buttons are all broken when no demographic no attached.

i think this is buggy. If i have a bunch of items in the pending docs, and skip the top ones, but assign a patient to like the 4th one, then the patient-specific buttons (tickler,echart,etc) become enabled on the top one! not on the item where i did the assignment on
