There are issues with Risk Factors in new ui:
- "Age at Onset" and "Life stage" fields are missing
- Assign issues field is not working
- Risk Factors as part of CPP is a default setting but when unchecked, it doesn't remove the CPP note
- Hide CPP (yes, no) radio buttons don't work
- Insert position drop box doesn't work
- In old ui the CPP note header is clickable and launches "Risk Factors as part of cpp History" to list all notes revision history. In new ui only Note Revision History can be viewed via encounter note rev_ link
- Archive button is only saving a new note, not archiving
Most of the issues (2-7) are applicable to other CPP notes in new ui and related to the bug [#3851].

3) we've elected to remove the checkbox altogether. there's no omd requirement for it, and it doesn't make sense to basically transform the cpp note into a progress note

4) This was confusing. The only functionlity that this box provides is to hide the CPP item from the print function (in echart)
I gave it a better label. "Hide from Print"

6) Not sure I fully understood this one. When you click the +, or any of the items in a list (like Risk Factors) then you get the modal which shows the list at the top (all the current risk factors). When you choose an item from the list, the form below gets populated with it's contents..and from on the top right side of the textarea for the note, is the rev. link where you can see previous revisions of that particular item. Is something missing?
I added the following feature. When you choose an item from the list (or load that particular one from the summary) then the item in the list is highlighted. Hopefully it helps navigate this inteface

7) I think we need to be a bit more specific about what archiving means here. When you archive a CPP in the new UI, it's removed from the list (like risk factors) but not from the middle section of the patient summary. This is similar to what happens in the classic UI where in the echart the middle section still shows the archived note, but the CPP section it belongs to does not.
When you click on the CPP header, you can see the archived note there as well.
ok, so I think I know what you mean in (6). The list doesn't show archived notes.
Update on the remaining issues in regards to Risk Factors CPP notes; re-tested using R15_Beta build # 218.

Observed: CPP Note is saved but not archived. In old ui new cpp note is archived.
