#3851 CPP modfiy modal issues

There are issues with the CPP modify/add modal.

1. insert position dosen't populate
2. add to cpp checkbox needs to be defaulted
3. archive dosen't archive

added popover onhover so you can read the entire truncated note without openning the modal.

fixing cpp note archiving:

1. archive doesn’t archive:
   Re-tested Master and Release_15_Beta -> Saved CPP note is archived.

However, observed that when creating a new CPP note, the Archive button is only saving the note, not archiving.

Adding CPP notes related issues:

1. Notes revision History doesn't show patient name (it does in old ui)
2. Assign issues field is not working
3. Ongoing concerns as part of CPP is a default setting but when unchecked, it doesn't remove the CPP note
4. Hide CPP (yes, no) radio buttons don't work
5. Insert position drop box doesn't work
6. In old ui the CPP note header is clickable and pops up Notes Revision History window. In new ui it only can be launched from the encounter note rev_ link.
