#3830 "Hand" (pointer) does not appear when hovering over links

When moving your cursor over most links in OSCAR15 cursor should turn into a pointer to indicate to the user that it is a link.

These commits just fix the top nav links but is a start.

in new ui the cursor should also turn into a pointer when hover over the following links/ tabs:

1. The Scratchpad shortcut icon next to the OSCAR messenger on the top navigation bar
2. User menu (logged in provider name) drop down (Settings, Support, Help)
3. 'Recent' tab at the top of the patient list (left-side menu) -> Hover over patient's name
4. Arrow down tab next to the 'Recent' tab at the top of the patient list -> drop down (Patient Sets, Caseload)
5. On the patient Summary page when hover over the rev # in encounter notes
6. All, Just my Notes, Just notes tabs
7. Consultation Request/ Response -> select patient/ provider from the search drop down or click on the clear button (cross buttons)
8. Forms tab on the patient menu -> Completed, Library tabs
9. Tickler page -> edit icon, cross (close)/validate button when edit is selected (ie Assigned to, Service date, Comments, etc.)
10. On the Ticker page -> Check All, Check None links
11. Create appointment -> select patient/provider name from the drop down list
12. Patient Search page (Select More results in Search drop down)
13. Patient Summary page Print encounter notes icon
14. Patient Summary Incoming section items (labs, docs)
