#3220 Hospital Billing module selects first referring physician in database if patient doesn't have one

Problem Summary:
If a patient doesn't have a referring physician listed in their Master File, the Hospital Billing module will select the first referring physician in the database when creating a hospital billing. If there's no referring physician assigned to the patient, the Hospital Billing module should simply leave the referring physician blank. Screenshot attached.

Expected Result:
The Hospital Billing module should simply leave the referring physician blank if there's no referring physician assigned to the patient.

Actual Result:
The first referring physician in the database is selected if a patient doesn't have a referring physician listed in their Master File.

Steps to Reproduce:
(1) Ensure that the patient you're testing with doesn't have a physician listed in the the "Referral Doctor" and "Referral Doctor #" fields of their Master File.
(2) From their Master File, click the "Hospital Billing" link in the menu on the left side of the screen.
(3) Notice that a referring physician is listed even thought the patient doesn't have one.
