#3189 Changing date on encounter note creates duplicate revision

Problem Summary:
If the "Encounter Date" of a progress note is changed while it's being entered, a duplicate note is created - as a revision to the note that's entered. Screenshot attached.

Expected Result:
A duplicate note should not be created - only the note that's typed in should be saved.

Actual Result:
A duplicate note is created.

Steps to Reproduce:
(1) Type an encounter note.
(2) Click the calendar symbol next to "Encounter Date" in the note.
(3) Change the encounter date to another date.
(4) Click the "Sign, Save and Exit" button.
(5) Reopen the encounter screen. The note will have two revisions.

Reproduced In:
12_1 Deb 362 (fresh install) + Ubuntu 12 LTS (fresh install)
