#3179 Providers are not listed in Simulation/ Generate OHIP File page

Precondition:

1. OSCAR McMaster trunk, system and DB up and running.
2. OSCAR user is registered and has all rights to login
3. Providers are created with Billing numbers

Steps to reproduce the problem:

1. Launch the browser and open OSCAR McMaster site
2. Log In
3. Select Administration from the top navigation bar
4. Once on Administration page, select Billing → Simulation OHIP File OR Generate OHIP File
5. Once on Simulation OHIP File/ Generate OHIP File, select provider from “Select Provider” drop down list

Expected Result:
Providers are listed and user is able to select a provider from the drop down list to create a report.

Observed:
Providers are not listed
