#3086 "Close" button on OHIP Report Simulation doesn't work

The close button for the OHIP simulation window doesn't close the window.

Steps to reproduce:

navigate to Administration -> Billing -> Simulation OHIP file
select care provider from the dropdown menu
select a start date - january 1 2013
select an end date - july 15 2013
click "create report"

---

Click "close" to close the window

Actual result:
Window does not close

Expected result:
Window should close
