#3053 Flagged Providers Text Field No Longer Displaying

Problem Summary:
After uploading a document with the Inbox Document Uploader, flagging a provider and demographic, the text field for the provider is no longer displayed and more providers can not be flagged.

Expected Result:
The flagged provider text field is displayed and works as intended.

Actual Result:
The flagged provider text field isn't displayed and more providers can not be flagged on the document. This occurs intermittently.

Steps To Reproduce:

1. Upload Document with Inbox Document Uploader
2. Enter Document Type, Description
3. Flag Demographic and flag provider
4. Click Save

Additional Information:
This is an intermittent issue and I haven't inspected the code for possibilities of paths that prevent the field from displaying
