#2281 Larry Kain Report counting non-visit billings

The Larry Kain report (under Visit Report in Admin) is counting billings not associated with a clinic visit (ie flu billing, bonus codes, etc) in its totals for visit numbers. This is artificially inflating the number of "visits" listed on the report. (Ex: For January, which contains a lot of flu billings, the total number of clinic visits on the standard visit report is about 6000. The LK report shows 13000 clinic visits.)

    Per clinical users, as suspected, the Larry Kain report should be using only bills associated with an appointment. Upon billing, users can choose a "Visit Type" and this is what should be used to set the categories on the LK report itself.
