#1853 "Nurse" select populated by doctors

On both the Oscar create demographic and master record screens, the Nurse selection control is populated with providers with role "Doctor", rather than "Nurse".

Steps to replicate:
1. Starting with a blank install, create both a doctor and nurse provider
2. Create a new demographic. Selecting "nurse" will allow choosing from doctors only
3. Edit the demographic. Selecting "nurse" will allow choosing from doctors only
