in the preventions section

when we enter that a preventative measure was done by "provider" "other", we
enter free text to specify who the "other" is.

it saves it as if the prevention was done by the current logged in
provider. not as the specified free text "other".

As of deb 370, selecting other provider adds the provider as system, system under the freetext area. Tested on 2 different installations
