This ticket is to track work to add a donation and sponsor form.
This is a form to manage donations. Donations should optionally allow contact information such as first name, last name, phone, email, company, address. The donation amount should be a free form box.
This donation page should have a link to allow the visitor to sponsor instead. Sponsorship will work very similar except have the sponsorship amount be a drop down list with fixed values. Similarly, the sponsorship page should have a link back to the donation page.
All contributions should be tracked to know when the contribution was made and how much was contributed.
