remove old windows dll's

only load tcl libraries when not in safe mode

Completed arguement work on ::oratcl::async_fetch which will become the
orafetch command before the final release.

update comments and remove test code

add oraparse, orabind, oraexec to root namespace

forgot to check oracle return status in Oratcl_Bind()

Inclusion of the new functions Oratcl_Parse, Oratcl_Bind, Oratcl_Exec.
The old functions Oratcl_Sql, Oratcl_Bindexec, and Oratcl_PLexec have
been rewritten to use the new functions.

The tcl command break must be referenced as ::break, because there is an
::oratcl::break command.
