Add flag to Oratcl_ColAppend so that it can choose between the col_list and
bind_list elements of the statement handle. Useful Oratcl_AsyncExec()
Fix core dump when Oratcl_AsyncParse() is called on an already used statement handle. Must call OCIReset().
