#5666 Display field "Include Header" checkbox should be closer to

Version : OrangeHRM 2.6.10

Description : Now it's not clear what the purpose of this check box is. I though if the check box is clicked, the fields will appear on the report, else they won't.

It's better to separate "Include Header" text from the group name, and have the checkbox immediately after "Include Header" text
