[WWdevel] preparing for WeBWorK 2.1.1

I've backported a good number of bugfixes from HEAD to rel-2-1-patches 
in preparation for tagging a 2.1.1 release. I've attached a list of the 
changes that have and have not been backported. Over the next few days, 
I'll be testing rel-2-1-patches to ensure that it is stable.

If you have some time, please test this branch as well so we can make 
sure nothing falls through the cracks. If you have a favorite bug you'd 
like to see fixed in 2.1.1, please fix it or let me know about it.
