I wasn't able to figure this issue out......

I decided to reinstall the whole thing on a new server and it is working
great now.

The box I was having trouble with has had a lot of development work done
on it, so there are a lot of remnants from past projects.

I appreciate the assistance you provided, keep up the great work.

Well, from the access.log it seems that apache is indeed sending the
file, i.e. this line

which is a 200Mb file. Hmm, are you sure it is not some kind of
antivirus or filter that is in between?

Have you tryed with a different browser (IE, FF) ?
