I have attached the contents of the apache log files from my latest test. There are no errors being generated in the error.log file when the unexpected event happens. I am using the latest PHP 5.3.1 CLI from http://windows.php.net (site appears down today). If you get back to the captcha it seems that it is somehow failing to send you the file. I have never tryed it on Windows, but are you using the cgi/cli or a module?
