Maybe a better question is - what is the supported Windows
installation??

Actually I have no experience with FastCGI on windows.
Maybe somebody else can help you on this one.
If there is some php log maybe I can track down better where the error
is. You could try enabling the error_reporting and display_errors to see

where php stops.
Open Upload itself was reported to be working on windows, dunno about 
IIS though.
I suspect some strange configuration error or something not supported, 
but I need more info.
