So as I understand the goal is to have akonadi plugin. Is the 0.4X engine working the same way as the 0.2X. I can then imagine trying to write a plugin that would sync a file with akonadi - using file-sync and akonadi-plugin

yes sure. I have a non syncml and a syncml nokia at home. The syncml is also kind of working with the current 0.2x engine and kde 3.5 (over bluetooth), but it still breaks and can only sync contacts - nothing else. So I would be glad if the opensync syncml part is working well.
