#4 packet detail page - add logging, info

- add "approved by" note in detail page

- add log for trace details:

a. Who uploaded trace, and when
b. Who approved or rejected trace, and when
c. Other actions... for example, if the trace was approved and then unapproved, or deleted, etc.

- enable moderators to comment/add notes when approving / unapproving
