[Openinteract-help] improper mime type set

When adding a document, I sometimes find that I can't view it or 
edit it after initially creating it.  I went to the database today
after one of these occurrences and found that mime_type was set to
'application/octet-stream' instead of 'text/html'.  When I updated
the database to change it, everything worked.  Any idea what might
cause this?
