Create a new evidence request and submit it to yourself.

Go into "To Do" and find the item.

Click on "Respond"

Click on "Add to attachments" and find and attach a file.

Click on "Submit response"

No error generated, but it refuses to save.

Click on "SAVE" and it is pointed out that there is a missing field. (response)

"Submit response" should point out that a required field is not filled out.
