#20 make Class.php honour a facility's submitter ID

This patch changes Claim_class.php so that it returns the facilities X12 submitter id from the facility table, instead of using the Employer Identification Number.
It depends on my previous patch, which allows one to enter facility specific X12 submitter IDs.
